ABSTRACT

The high incidence of cardiovascular diseases is a serious threat to human health, and endovascular surgery has become the standard treatment for most interventional cardiovascular diseases. The robotassisted endovascular surgery system further enhances surgeons' ability to perform minimally invasive endovascular procedures in interventional cardiology. This study presents a new robotic technique for coronary intervention from the perspective of clinical application. Aiming at clinical application scenarios, this scheme proposed an intuitive guide wire catheter mechanism design, which accurately and perfectly simulates the doctor's hand movements, realizes the positive and negative direction translation of the guide wire catheter, accurate torque control of the guide wire rotation and locking. The results of animal test showed that the R-OneTM has a high degree of dexterity, accuracy and stability, and meets the clinical needs.

ABSTRACT

The pathological hallmarks of psoriasis involve alterations in T cell genes associated with transcriptional levels, which are determined by chromatin accessibility. However, to what extent these alterations in T cell transcriptional levels recapitulate the epigenetic features of psoriasis remains unknown. Here, we systematically profiled chromatin accessibility on Th1, Th2, Th1-17, Th17, and Treg cells and found that chromatin remodeling contributes significantly to the pathogenesis of the disease. The chromatin remodeling tendency of different subtypes of Th cells were relatively consistent. Next, we profiled chromatin accessibility and transcriptional dynamics on memory Th/Treg cells. In the memory Th cells, 803 increased and 545 decreased chromatin-accessible regions were identified. In the memory Treg cells, 713 increased and 1206 decreased chromatin-accessible regions were identified. A total of 54 and 53 genes were differentially expressed in the peaks associated with the memory Th and Treg cells. FOSL1, SPI1, ATF3, NFKB1, RUNX, ETV4, ERG, FLI1, and ETC1 were identified as regulators in the development of psoriasis. The transcriptional regulatory network showed that NFKB1 and RELA were highly connected and central to the network. NFKB1 regulated the genes of CCL3, CXCL2, and IL1RN. Our results provided candidate transcription factors and a foundational framework of the regulomes of the disease.

ABSTRACT

Esculetin, a natural derivative from the traditional and widely-used Chinese medicinal herb Cortex Fraxini, has a variety of pharmacological effects, especially in anti-inflammation. However, it is not clear whether esculetin has a therapeutic effect on sepsis. This study aimed to investigate the anti-inflammatory and protective effects of esculetin on early sepsis. The results showed that the lung injury was significantly relieved with the treatment of esculetin, accompanied with the restrained production of inflammatory factors including IL-1β, IL-6, TNF-α, CCL2 and iNOS during the early phase of E.coli-induced sepsis. Of note, activation of NF-κB and STAT1/STAT3 signals, the main upstream signals of many inflammatory factors, were attenuated by esculetin in both lung tissues from septic mice and LPS-stimulated macrophage. These findings suggested that the protection of esculetin against early sepsis should be related to its anti-inflammatory effect, which was at least partly due to its inhibition on NF-κB and STAT1/STAT3 signaling pathway in macrophage. Thus, esculetin could serve as a potential therapeutic agent by rebalancing innate immune response in macrophage for the treatment of early sepsis.

ABSTRACT

Objective: To explore the application value of enhanced recovery after surgery (ERAS) with the multidisciplinary team (MDT) model in laryngeal cancer surgery. Methods: Eighty patients with laryngeal cancer treated in Department of Otorhinolaryngology Head and Neck Surgery of Yantai Yuhuangding Hospital from May 2016 to June 2017 were selected, including 76 males and 4 females, aged 45 to 75 years old. By random number table method, they were divided into ERAS group (40 cases) and control group (40 cases). Visual analogue scale (VAS), general comfort questionnaire (GCQ) and self-rating Anxiety Scale (SAS) were used to evaluate the symptoms and signs and psychological state of the two groups before and after operation. Mann Whitney U test was used for non-normal distribution data, and chi square test, Fisher exact probability method and covariance analysis were used for classification data. Repeated measures analysis of variance was used for the comparison of each group at different time points. Results: Two cases in the ERAS group and six cases in the control group withdrew from the study for some reason. Finally, 38 cases in the ERAS group and 34 cases in the control group were enrolled in this study. The postoperative pain scores of the two groups were the highest at 6 h after operation, and then gradually decreased. At different time points after operation, the pain scores of ERAS group were lower than those of the control group. At 24 h after operation, the pain relief degree of ERAS group was significantly higher than that of the control group, with a statistically significant difference (P<0.05). Compared to control group, ERAS group had lower preoperative thirst score [(0.15±0.36) vs. (4.29±1.17), Z=-7.695, P<0.001] and hunger score [(0.38±0.49) vs. (3.44±1.13), Z=-7.426, P<0.001]. The total number of postoperative adverse reactions (8 vs.16), oral feeding time [(4.06±4.42) d vs. (9.06±2.42) d] and postoperative hospital stay [(5.91±0.97) d vs. (11.03±2.11)d] in ERAS group were lower than those in control group (statistics 5.461, -4.558, -7.347, P<0.05), but there was no significant difference in postoperative catheter indwelling time and neck drainage tube indwelling time between the two groups (P>0.05). Before discharge, the comfort of ERAS group was significantly higher than that of control group [(60.37±8.78) vs. (50.38±8.08), Z=-4.370, P<0.001]. Before discharge, the anxiety level of ERAS group decreased, while that of the control group increased significantly, which was higher than that of ERAS Group [(59.12±6.43) vs. (52.62±6.25), Z=-4.179, P<0.001]. Conclusion: The application of multidisciplinary ERAS in laryngeal cancer surgery can improve preoperative hunger and thirst, postoperative pain and mental state, shorten the length of hospital stay and reduce postoperative adverse reactions.

ABSTRACT

Objective@#To evaluate the efficiency and safety of transurethral holmium laser enucleation of the prostate (HoLEP) in the treatment of BPH in patients with a history of transrectal prostate biopsy (TRPB).@*METHODS@#We retrospectively analyzed the clinical data on 102 cases of BPH treated by HoLEP in our hospital between November 2015 and May 2017, of which 42 had received TRPB prior to HoLEP (the PB group) but not the other 60 (the non-TRPB [NPB] group). We compared the preoperative, perioperative and postoperative follow-up data between the two groups of patients.@*RESULTS@#There were no statistically significant differences in the mean age, prostate volume, and preoperative post-void residual urine volume (PVR), IPSS, quality of life (QOL) score and maximum urinary flow rate (Qmax) between the two groups of patients. The preoperative PSA level was significantly higher in the PB than in the NPB group ([10.30 ± 3.62] vs [2.62 ± 1.75] μg/L, P < 0.01), and the operation time markedly longer in the former than in the latter ([78.00 ± 18.25] vs [67.93 ± 15.89] min, P < 0.01), particularly in the patients with an interval of <2 weeks between HoLEP and TRPB than in those with an interval of ≥2 weeks ([91.17 ± 16.51] vs [68.13 ± 12.45] min, P < 0.01). Statistically significant differences were not found in the postoperative hemoglobin level, continuous bladder irrigation duration, catheter-indwelling time and hospital stay, nor in the incidence rate of transient urinary incontinence between the PB and NPB groups (47.62% vs 45%, P = 0.794). There were no transurethral resection syndrome, bladder or rectal injury, or blood transfusion in either group, nor statistically significant differences in PVR, Qmax, IPSS and QOL score between the two groups of patients at 3, 6 or 12 months after operation.@*CONCLUSIONS@#HoLEP is a safe and effective surgical treatment of BPH for patients with a history of TRPB, which can reduce the time and increase the safety of operation when performed at ≥2 weeks after TRPB.

ABSTRACT

The research for efficient and low-cost electrocatalysts for water splitting are essential for the exploitation and application of hydrogen energy. Transitional metal phosphides are considered as one of the most promising bifunctional water splitting electrocatalysts. Herein, we reported a facile two-step method (firstly hydrothermal preparation, then phosphorization under low temperature) to synthesize the bead-chain like nanoarrays of CoP supported on three dimensional Nickel foam (CoP/NF). The as-prepared CoP/NF could act as an efficient bifunctional catalyst for overall water splitting. The catalyst exhibited remarkable activity for both the oxygen evolution reaction (OER) and hydrogen evolution reaction (HER) in alkaline media, delivering a current density of 10 mA/cm2at an overpotential of 281 mV for OER and 95 mV for HER, respectively. Efficient water splitting in alkaline system was realized by applying a two-electrode system with the CoP/NF acting as both the anode and cathode. The applied potential was 1.63 V to obtain the current density of 10 mA/cm2,and good stability was also testified.

ABSTRACT

BACKGROUND: The incidence of ankylosing spondylitis (AS) presents a trend of rising year by year, accompanied by a higher disability. Therefore, fully understanding the related factors to the development of AS is of great significance to develop a rational treatment scheme.OBJECTIVE: To evaluate the expression levels of Dickkopf-1 and sclerostin in Wnt signaling pathway as well as vascular endothelial growth factor A (VEGF-A) in the AS patients, and to evaluate their correlation with AS progression.METHODS: AS patients and healthy volunteers were recruited, and their baseline data and medical history were collected. The expression levels of Dickkopf-1, sclerostin, VEGF-A and C-reactive protein in the peripheral blood were detected; the AS progression was evaluated by Bath AS Disease Activity Index and Bath AS Functional Index; the imaging performance was assessed by modified Stroke AS Spine Score; the correlation of Dickkopf-1, sclerostin and VEGF-A with AS progression, imaging performance and inflammatory reaction was analyzed by Spearman's rank correlation analysis and multiple linear regression analysis.RESULTS AND CONCLUSION: No significant differences were found in the expression levels of Dickkopf-1, sclerostin and VEGF-A between two groups (P > 0.05). The AS patients without syndesmophyte and with higher erythrocyte sedimentation rate and C-reactive protein level appeared with an significant increase in the Dickkopf-1 level (P < 0.05),which was significantly related to sclerostin level (r=0.592, P=0.000). The As patients with the history of smoking, increase in erythrocyte sedimentation rate and C-reactive protein, as well as higher Bath AS Disease Activity Index and Bath AS Functional Index presented with a higher level of VEGF-A. Multiple linear regression analysis showed that erythrocyte sedimentation rate, C-reactive protein, syndesmophyte and sclerostin level were the independent factors affecting the Dickkopf-1 level (P ≤ 0.001); the history of smoking, erythrocyte sedimentation rate, and C-reactive protein were the independent factors affecting VEGF-A (P < 0.005). These results suggest that in AS patients, the Dickkopf-1 level is related to syndesmophyte and systemic inflammatory response, while the history of smoking affects VEGF-A level. Therefore, all above indicators can be used to evaluate osteophyte formation and bone mass loss.

ABSTRACT

Tumor cell plasticity, including epithelial to mesenchymal transition (EMT) and its reverse program, mesenchymal to epithe-lial transition (MET), regulates circulating tumor cells and carcinoma metastasis. Twist is overexpressed in rhabdomyosarcoma, breast cancer, gastric cancer, and other tumors. Twist, as a transcriptional factor, cross-talks with multiple signaling pathways, forming a com-plex network to participate in the regulation of EMT/MET in circulating tumor cells, which in turn promotes metastasis of tumor cells. Therefore, monitoring the level of Twist and epithelial–mesenchymal phenotypic molecules is important as it may be beneficial for in-creasing the detection ratio of circulating tumor cells as tumor biomarkers and for evaluating the effects of anticancer drugs.
